I think it's more plausible to believe that he IS in the loop, approves the decision, subsequently gets negative feedback that makes him change his mind, reverses the decision, and lies about it.

A president should be able to walk and chew gum at the same time. A war time president not paying attention AND crapping on the first amendment and the free press at the White House Correspondents dinner should most certainly be called out for both. A President is lucky if they can get three or four big things through, in a first term. In a second term they start out as a lame duck, who can still fight, until after the mid-terms, and then it is trying to hold the fort against all attacks. This takes thought, endurance, attention, and detail, please tell me which, if any, of those, President Trump has for promoting a policy, that can get enacted via the Congress.
Previous administrations sent out trial balloons. This administration seems to make "official" policies and treat them as trial balloons. Not sure if this makes a difference because it has become the norm.
